I have read and agree to the Terms and Conditions for Voluntary Contribution to the Kongsi Land Preservation Reserve.

I hereby declare that the funds I am contributing are derived from lawful and legitimate sources, and I understand that any false declaration may result in rejection of the contribution and reporting to the relevant authorities.

I hereby give my consent to Koperasi Kongsi Selangor Berhad ("Kongsi" or "Kongsi Co-op") to the collection of your personal details for record-keeping and compliance purposes.
